The Jennifer Jaff CareLine for Patients with IBD provides specific case management services to patients and/or their providers. The CareLine is staffed by a team of professional case managers with both nursing and social work backgrounds, and non-clinical case managers who have an insurance industry background with coding and billing expertise. Using their knowledge of reimbursement issues, this dedicated team will be able to help patients diagnosed with Inflammatory Bowel Disease with the following:

  • Provide direct assistance with enrollment in Medicare (B and D) and Medicaid, as well as Supplemental Security Income (SSI) and Social Security Disability (SSDI) programs
  • Provide education and assistance with the disability process, including education, application and appeals
  • Assist with enrollment in employer-initiated programs, including the Family Medical Leave Act (FMLA), short-term disability insurance, long-term disability insurance, and COBRA
  • Educate patients on employment rights and ways to access legal services when rights are denied
  • Help identify solutions that reduce medical debt challenges by networking patients to all known local, state and/or national resources for assistance
  • Navigate through the reimbursement system, including initial interpretation of the patient’s benefit language
  • Assist with insurance appeals, coding and billing errors and clarification of plan benefits
  • Negotiate access to prescribed healthcare services for both insured and uninsured patients
  • Provide case management services to uninsured patients, ensuring access to care and enrollment into Medicaid, a Marketplace plan and/or charity care program, when appropriate
